Moving outside the HMO coverage area
If you move out of a plan service area or will be away for more than two months, you and your eligible family members must transfer into a different plan available in your new location. If you later return to your original location, you will have a Period of Initial Eligibility during which you can re-enroll in your original plan if you choose

During the Qualifying Life Event, you may also enroll any other eligible family members not already enrolled in your UC-sponsored health plans. The Period of Initial Eligibility (PIE) generally starts on the first day of the qualifying event and ends 31 days later.
Good to Know
After enrolling the new family member, employees must complete the Family Member Eligibility Verification process. If you do not respond by the deadline, your new family member will lose their coverage, and UCPath will disenroll them from the plans. See details under each Qualifying Life Event.

Changes are made to the UC Pension and Life insurance plans at UC Retirement At Your Service (UCRAYS).
403B, 457B, and Defined Contribution plan go to my UC retirement website.
Health Savings Account beneficiaries are done with HealthEquity. Please call 866-212-4729 for help.
